Amatya YR, Sapkota D. Patterns of Delivery and Perinatal Outcomes Among Women Delivered at District Hospital of Rural Nepal. |
Abstract: Studies have shown increased rate of labour induction and cesarean section (CS) with several health consequences for neonates and mothers. Still there is limited data especially in developing countries. A hospital based retrospective cross sectional study was done where all hospital deliveries occurring during the period of 6 months were recorded to assess the patterns of deliveries and their associated perinatal outcomes. Cesarean rate was 18.5%, with meconium- stained liquor being the commonest reason, and 12.5% of deliveries were medically induced. Post dated pregnancies were more likely to have either elective CS or medical induction. Medically induced cases were less likely to have meconium stained liquor however, these cases are at higher odds of emergency CS. Induction of labour is not always without risk as cases lead to more emergency cesarean section. However, this paper highlights that medical induction of labor improve neonatal outcomes which might be attributed to timely intervention in such cases. Mayura S, Suguna E, Dongre AR. Perceived Cause and Effect Relationship for Nutritional Anemia Among Adolescent Girls in Rural Puducherry. |
Abstract: Background & Objectives: The prevalence of nutritional anemia among adolescent girls is high (47.6% to 90%) as previously reported in the different parts of North India. Since there are limited studies on nutritional anemia at the sub-district level in the Southern part of India, there is a need to know the extent of the problem among adolescent girls to plan context specific interventions. With this background, the present study was done with the objectives 1) To find the prevalence of nutritional anemia among adolescent girls 2) To ‘qualitatively’ explore their perceptions of the causes and effects of nutritional anemia. Methods: The study was undertaken in two wards of Kallitheerthalkuppam village, the field practice area of the Department of Community Medicine, SMVMCH, Puducherry. Mixed-methods design which includes quantitative (survey) and qualitative (cause-effect diagram) methods was used. A representative sample of 100 adolescent girls aged 12-19 years were the study participants. Data for quantitative (survey using a questionnaire) and qualitative (cause-effect diagram) methods were collected over the period of two months. Ethical clearance was obtained. Quantitative date was entered and analyzed in Epi-Info software package version 3.4.3. Frequencies, percentages and mean ±SD were calculated. Cause and effect diagram was obtained from the qualitative data and analyzed manually. Result: The prevalence of anemia among the study population was 58 per cent. In The cause and effect diagram exercise, girls linked deficiency of iron as the cause for tiredness, irregular menstrual cycles and low birth weight. They also linked deficiency of iron and the habit of not wearing footwear to tiredness, reduced intake of green leafy vegetables, and intestinal worm infections to giddiness and hookworm infections to peptic ulcer. Interpretation & Conclusions: The study found a gap between knowledge and practice as reflected in the cause and effect diagram. This result indicates the need to initiate an effective behavior change communication for improving the hemoglobin status of adolescent girls. Kirubakaran S, Dongre AR. Prevalence and Determinants of Tobacco Usage Among Youth (Age Group 15-24) in Peri-Urban area of Villupuram, Tamilnadu. |
Abstract: Objectives:The objectives were to study the prevalence and pattern of tobacco use among youth (15-24 years) and to explore the various determinants of tobacco usage. Material and Methods: It was a community-based cross sectional study. Based on the Global Adult Tobacco Survey 2008-10, the prevalence of 34.6% was used to calculate the sample size. A representative sample of 610 youth was selected and interviewed using pre-designed and pre tested questionnaire by doing house to house visits. The data was entered and analyzed using the Epi_info software package. Result: About 57.4% males and 6.5% females gave the history of ever usage of tobacco products, 48.6% of males and 0.81% of females had consumed any tobacco products in last 30 days. Among male out of 177, 88.1% smoked cigarette and only two females have consumed dry tobacco leaves. The major reason to use were peer pressure (96%) and media influence (59.2%). The common occasion to consume tobacco products were stress (77.6%) and to cheer up (60.3%). Conclusion and recommendations: The prevalence of current use of any tobacco products among youth (15 to 24) was found to be very high. Prevention of tobacco consumption in youth reduces/delays the incidence of non communicable diseases in their adulthood. Key words: Tobacco, Prevalence, Youth, India |

Mohite SL, Ghorpade MV. A Study of Metallo Beta Lactamase Producing Clinical Isolates of Pseudomonas aeruginosa. |
Abstract: Introduction: The emergence and rapid spread of multidrug resistant Pseudomonas aeruginosa causing nosocomial infections is a great concern worldwide. Detection of MBL is also a challenge for routine microbiology laboratories, since there are no standardized methods for MBL detection. The present study was conducted at our hospital with an aim to know the prevalence of metallo beta lactamase producing Pseudomonas aeruginosa, to know MIC of various antibiotics, transfer of drug resistance and predisposing factors for infection with MBL strain. Material and Methods: A total of 608 clinical isolates were tested for the presence of metallo beta lactamase enzyme from Oct 2008 to Sept 2013. Imipenem resistant Ps. aeruginosa isolates were selected for the presence of MBL by Imipenem – EDTA combined disc test, Imipenem – EDTA double disc synergy test (DDST), EDTA disk potentiation test using Ceftazidime, Ceftizoxime, Cefepime, Cefotaxime and MBL E- Test. Result: Out of 608 clinical isolates 91 (14.96%) were resistant to imipenem and 81 (13.32%) were MBL positive by initial two tests. 76(83.51%) were positive by EDTA disk potentiation test using Ceftazidime, Ceftizoxime, Cefepime, Cefotaxime and 63 (69.23%) were positive by E- test. It is observed that 62/81(76.54%) of MBL producing Pseudomonas isolates could transfer the resistance of imipenem to the recipient E. coli J53 AZR strains at 370C. The prevalence of MBL producing Pseudomonas aeruginosa was 81/608 (13.32%). Conclusion: The early detection of MBL producing Pseudomonas aeruginosa may help in appropriate antimicrobial therapy and avoid the development and dissemination of these multidrug resistance strains. Hence all Pseudomonas aeruginosa isolates resistant to imipenem should be screened for MBL production.

Abstract: We report a case of multiple anatomical variations in the right gluteal region of a 60 years old male cadaver. In the present case lower formation of the sciatic nerve was observed with the peroneal component emerging between the two parts of piriformis along with the inferior gluteal nerve and the tibial component arising inferior to it accompanied by the posterior femoral cutaneous nerve. At the lower border of piriformis, both the tibial and common peroneal components united and descended as a single trunk and further had a normal termination in the popliteal fossa. The posterior femoral cutaneous nerve (PFCN) also presented variations by forming a communicating plexus with the inferior gluteal nerve and terminated in the gluteal region. The variations in the formation of sciatic nerve, the anomalous communication and termination of the PFCN are very important for surgeons and clinicians while planning the treatment regime. Anaesthetists should also be aware of these variations during nerve blocks.

Abstract: We report a case of malignant nodular hidradenoma in an old woman, who presented with a nodular swelling in the right side of nose near the medial canthus of the right eye. Wide excision of the nodular mass with a clear margin of healthy surrounding tissue was performed along with primary closure. Post operatively, adjuvant radiation therapy was given on a telecobalt machine due to the presence of high risk features. In general, malignant forms of hidradenomas are not usual and treatment strategies should be individualized.

Abstract: Growing Teratoma Syndrome (GTS) is an unusual sequelae of nondysgerminomatous germ cell tumours. Here, we report the case of a lady with GTS presenting in a unique manner, as sciatica. The predisposing factors for development of GTS are advanced nature of primary tumor, mature elements in primary tumor and incomplete primary surgery. Prompt and timely excision of the tumour results in complete excision and symptomatic relief, in addition to confirmation of diagnosis and exclusion of malignancy.
