Abstract: Objective: To assess the awareness on newly initiated government schemes, to find the utilization of those, to find the association between awareness on newly initiated government schemes and selected variables and also the association between utilization of newly initiated government schemes and selected variables. Method: A descriptive survey design was adopted for the study and 300 people residing in the villages of Udupi Taluk were selected by convenience sampling technique. The data were collected by administering semi-structured and structured knowledge questionnaire on newly initiated government schemes and about its utilization. Results and Conclusion: The study results revealed that about 66.7% participants had average knowledge, 27% had good knowledge and 6.3% had poor knowledge. Only 27% of the participants were utilizing these services. There was significant association between awareness on newly initiated government schemes and selected variables such as monthly family income, educational qualification and occupation and there was significant association between utilization and the selected variables such as occupation. The study concluded that even though people are aware about newly initiated government schemes, they may not utilize the services. So there is a need to motivate the people to utilize these government services by creating awareness among people.

Pai MS, Lewis LE, Yashoda S, Priyadarshini S, Margaret B. Mortality and Morbidity Among Preterm Neonates in Neonatal Intensive Care Unit. |
Abstract: The aim of the study was to determine the mortality and morbidity among preterm neonates admitted to Neonatal Intensive Care Unit (NICU) of a tertiary care hospital of Udupi District, Karnataka, South India. Material and methods: A cross sectional study was carried out among preterm neonates born with gestational age less than 37 weeks. A total of 1194 preterm neonates admitted to NICU in the year 2014-2015 were included in the study. Result: Among 1194 preterm neonates, 54.3 % were boys. Maximum of the preterm neonates (51.3 %) were within the gestational age of 30-34 weeks. There were 10.4% of preterm neonates whose birth weight was less than 1000 gms. The survival rate of preterm neonates was 92.5 %. Respiratory distress syndrome, sepsis, Hyperbilirubenemia and perinatal asphyxia were the major morbidities among the preterms admitted. Conclusion: Preterm births are one of the condition that require maximum attention and care.

Gaps in Infection Control Practices of Tuberculosis Among the Patients Attending Designated Microscopy Centre (DMC): Mixed Method Study. |
Abstract: Introduction: The purpose of the study was to find the knowledge, attitude, and practice of infection control of tuberculosis among presumptive tuberculosis cases and the infection control practices actually followed by TB patients from the Tuberculosis Health Visitor (TBHV). Material and Methods: An exploratory sequential design constituting of quantitative methods (Hospital based cross sectional survey) among Presumptive TB cases referred to DMC centre followed by qualitative methods (Focus Group Discussion [FGD]) with TBHV was used. Results: Out of 422 Presumptive TB cases participated in the study, 305 (72.3%) were males and 117 (27.7%) were females. The mean age of the respondents was 52.03 (±15.77 standard deviation [SD]) years and 202 (47.9%) were illiterate. About, 195(46.4%) participants were able to demonstrate good cough etiquette and 167 (39.6%) knew how to dispose sputum. In multiple logistic regression analysis used to identify independent variables that predict the practice of demonstration of cough, the odds of demonstrating correct cough etiquette is significantly higher: 1.02 times (CI: 1.01-1.03), 1.91 times (CI: 1.08-3.37) and 1.429 (CI: 1.05-1.94) in participants of age less than 45 years, those who have completed primary education, and those who have the habit of using a hand kerchief while coughing respectively, when compared to their counterparts. Conclusion: The advocacy communication strategies related to infection control should be focussed at school level and also to immediate care givers and household members of the pulmonary TB and presumptive TB patients.

Kistan K, Moodley Y. Are Abnormal Pre-operative Platelet Counts a Risk Factor for Major Adverse Cardiovascular Events Following Non-cardiac Surgery? |
Abstract: Background: We sought to determine whether abnormal pre-operative platelet counts are associated with increased major adverse cardiovascular events (MACE) in South African (SA) non-cardiac surgery settings. Methods: This was a propensity matched case-control study of data from 150 SA non-cardiac surgery patients. Patients had their data (age, gender, cardiovascular risk factors and pre-operative platelet counts) collected as part of a pre-existing registry. Cases were patients with post-operative MACE (post-operative stroke, myocardial infarction or all-cause mortality). Controls were patients without post-operative MACE. Data were analysed using conventional methods for matched case-control studies. Results: Our most important finding was that the odds of post-operative MACE were six-fold higher in patients with pre-operative thrombocytopenia (low platelet count) when compared to patients with normal pre-operative platelet counts (Odds Ratio, OR: 6.28, 95% Confidence Interval, CI: 1.19-33.12; p=0.031). Conclusion: Thrombocytopenia is a risk factor for post-operative MACE in SA non-cardiac surgery patients.

Abstract: Background: Usability assessment is necessary to know how effective is the implemented Hospital Information System (HIS) integrated Electronic Medical Record (EMR). The assessment explores the factors that influence the satisfaction level of the end-user. Objective: To understand the end user attitude towards HIS integrated EMR, to assess the satisfaction level of the end-user and to analyze the challenges and limitations. Methodology: A descriptive cross-sectional study was carried out among nursing, billing, front office, medical records department and paramedical staff of a super specialty hospital in Southern India. A structured and validated questionnaire used for the data collection. Results: The study result evident 61.40% of staff with a moderate level of satisfied with the present HIS integrated EMR. Post Hoc test revealed a significant difference in the attitudes of staffs towards HIS integrated EMR. The major technical challenges reported by the end users were; slow network, poor integrity, difficulty in accessing data, no guidelines for using HIS and not the friendly user interface. The lack of training, lack of healthcare professional’s support, lack of experience in using the system and scared to use the HIS integrated EMR was the human challenges expressed by the end-users. The information availability, software, and communication system were the limitations that restricted the end-users to utilize the system fully. Conclusion: The study indicated that the better acceptability of HIS integrated EMR is only possible through the; involvement of end-users into the mainstream of implementation and regular evaluation of end-user satisfaction. This approach will provide the scope for improving the system to meet the end-user requirement and lead to better sustainability.

Andrade LS, Kalthur SG. Topography of Wormian Bones in Cadaveric Dry Skulls. |
Abstract: Apart from the normal cranial bones, a few supernumerary/ wormian bones can be seen in dry skulls. This study intends to find the presence of wormian bones in the skulls pertaining to the coastal region of south India. Eighty-eight dry adult skulls were observed for the wormian bones. The results show that sixty-two skulls had wormian bones. Majority of these were observed at the lambdoid suture. The presence of such bones are of great clinical importance to the radiologists and the neurosurgeon.

Abstract: Background: The structures passing through the obturator foramen are usually arranged as follows from above downwards – Obturator nerve, obturator artery and obturator vein. But there can be a change in this pattern which is very important for the pelvic surgeons. Aim: To study the origin of the obturator artery and to note the arrangement of the structures passing through the obturator foramen. Materials and Method: 33 formalin-fixed adult pelvises of unknown age were studied to note the origin of obturator artery and the structures passing through the obturator foramen. Results: Many specimens had variation in the structures passing through the obturator foramen. Conclusion: Knowledge of structures in the pelvis is a must for every surgeon performing surgery in Bogros space and also for herniorrhaphy.

Covantev S, Mazuruc N, Belic O. Kimmerle Anomaly – An Important Anatomical Variation. |
Abstract: The anatomy of the first vertebra, namely atlas has significant clinical implications. One of the clinically significant anatomical variants is the presence of an arcuate foramen. Arcuate foramen of atlas is an ossification of the oblique atlanto-occipital ligament superior to the vertebral artery groove of the atlas. The vertebral artery, which passes under these bony projections, can be compressed along with the suboccipital nerve causing a variety of symptoms. We present a series of anatomical case reports from a collection of 50 atlases with their analysis and brief review of their clinical significance. This anomaly is an underestimated structure, which is important to take into account in case of patients with a headache, vertigo, shoulder and arm pain since in case of complete arcuate foramen the number of symptomatic patients can be up to 78%.

Kamath BJ, Madegowda A, Sujir P. Modified Thomas Splint for the Non-operative Management of Pediatric Femoral Shaft Fractures. |
Abstract: Pediatric femoral fractures constitutes less than 2% of all fractures in children and management of these fractures either by conservative or operative methods are determined by many variables. Initial traction followed by delayed hip spica application is one of the non-operative treatment option for femoral shaft fractures in children below 6 years of age. Selection of an appropriate size Thomas splint to provide an effective initial traction plays a vital role in this treatment aspect. But, there is a paucity of commercially available Thomas splints of appropriate size for pediatric population. As a solution to address these problems, we share our experience of modified Thomas splints which are made using some of the easily available and reusable parts of Ilizarvo apparatus.

Mallya SP, Sujir P. Recurrence of Chondromyxoid Fibroma of Great Toe. |
Abstract: Chondromyxoid fibroma (CMF) is a benign cartilaginous tumour which involves mainly the long bones and proximal tibia being common. CMF of foot and phalanx is very rare. Various treatment options are available for this tumour and recurrences have been reported following surgery. 24 year old male was treated with curettage and bone grafting for chondromyxoid fibroma of great toe 8 years back. 6 years later, he presented with local recurrence of tumour with extensive soft tissue involvement. He was treated with amputation of great toe. After Two years follow up, patient was symptom free.
